There are three ways to become an All On 4 Plus® provider:
REFERRING DENTIST OR SPECIALIST
A referral to All On 4 Plus® Premium Provider ensures that your patient is seen by trained experts in implant-based full arch rehabilitation and receives the best possible care under General Anaesthetic at a dedicated facility.
The results are instant, the teeth are final and recovery is typically quick and uneventful. Your patients’ results and overall experience will ultimately reflect on you, as the referring dentist, and would typically help your own professional growth.
The consultation fee (for referrals network patients) is $180 and includes a CBCT (if required) at no additional out of pocket cost (rebate only).
After the consultation your patient will be assigned a treatment coordinator who will provide them with a treatment proposal and quote along will all the relevant information, tests and prescriptions as required.
The surgeon and anaesthetist will consult your patient (again) on the day of their surgery.
Your patient would typically receive a set of printed teeth on the same day (or next morning) followed by Immediate Final Teeth® (with a milled titanium frame) within 24-72 hours of their surgery.
Your patient will be returned to your care for maintenance and you will receive any support that you might require during their maintenance phase.
Frequent Questions
Most of our All On 4 Plus® Premium Providers offer online consultations for referred patients who come from afar, at which you (as the referring dentist) may also participate. The final treatment will be subject to a further assessment of your patient’s medical/dental history and circumstances, as well as a clinical examination on the day of surgery.
Depending on the surgeon and facility, in most cases you will be welcome to attend the surgery of your patient.
How To Refer Your Patient?
Please fill out the patient referral form follow the prompts to refer your patient and to attach any relevant picture or x-rays. Your chosen clinic/surgeon will be notified and will contact your patient to arrange for an appointment.
Your patient will receive some online forms in advance of their appointment to ensure the most streamlined process and that nothing is missed.
