An Affiliate is a referring dentist or specialist who enters a formal Affiliation Agreement and is subsequently trained by Dr Fibishenko or another Premium Provider.
An Affiliate is a referring dentist or specialist who enters a formal Affiliation Agreement and is subsequently trained by Dr Fibishenko or another Premium Provider to undertake the following components of All On 4 Plus® system:
As an Affiliate dentist you will bill your patient directly for the above components of treatment which you will perform (subject to training).
There is no cost to being an Affiliate. It is subject to a 5-year Affiliation Agreement.
As an affiliate you will have a limited license to All On 4 Plus® when referring specifically to your role as an Affiliate or to the system or treatment process in a manner prescribed under the Affiliation Agreement.t.
Cost-free Affiliate Status is subject to you working with a Premium Provider of your choice for the surgical component of the work, and on the basis of a minimum of 5 referrals per year.
